OUR SURGERY IS NOW LIVE FOR SUMMARY CARE RECORDS

This is a national sharing agreement whereby in an emergency healthcare staff involved in your care can access limited information only regarding your medication and any allergies. Unless you are unconscious you will always be asked for your consent to access this information.

Summary care records are stored securely on NHS computers. There are measures in place to ensure the information is stored safely, stays private and can only be looked at by those authorised to do so. Please be aware this information will not be sold or share to third party company's.

Opting out

If you do not wish for your information to be accessible you can opt out by filling out a form that is available at reception.

You can also print an opt out form and pass it in to the surgery. Please click here.

You can opt back in for this database if you choose to.

HSCIC - Health and Social Care Information Centre

Information about you and the care you receive is shared, in a secure system, to help plan and improve services for all patients. They will link information from all the different places where your receive care, such as your GP, hospital and community service, to help provide a full picture. This will allow them to compare the care you received in one area against the care you received in another, so they can see what has worked best. Information such as your postcode and NHS number, but not your name, will be used to link your records in a secure system, so your identity is protected. Information which does not reveal your identity can then be used by others, such as researches and those planning health services, to make sure we provide the best care possible for everyone.

Medical staff treating you in GP surgeries, hospitals, A&E and out of hours centres will not use or be able to use this database. However the uploaded data is likely to be made available to organisations outside of the NHS, such as, universities and commercial organisations.
