Various non-NHS services attract a fee, examples include; employment medicals, private sick notes and private insurance forms.

Our list of non NHS fees is as follows: 

• Private sick note for employer £15
• Fitness to perform (stage/tv etc) £15
• Holiday cancellation certificate £20
• Fitness to travel certificate £20
• Bupa/PP etch claim form £20
• Medical report without examination £30
• Taxi/HGV medical £92.50
